Job summary

Boost Home Healthcare - Northern Colorado is seeking a dedicated Occupational Therapist to provide skilled home health services in Fort Collins, Colorado. You will assess patients, develop plans of care, and coordinate treatment with a care team to promote independence and recovery.

The ideal candidate holds an ACOTE-accredited OT degree, is licensed in the state, and has experience in a home health or acute care setting. This full-time role offers comprehensive benefits and growth opportunities.

Qualifications

• OT degree from ACOTE-accredited program
• Licensed to practice occupational therapy in the state of Colorado
• Minimum one year in home health or acute care preferred

Responsibilities

• Provide prescribed occupational therapy services in patient homes.
• Develop and monitor plans of care; adjust every 30 days as needed.
• Collaborate with physicians and other team members on patient care.

What you’ll be doing:

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and all skilled procedures.
• Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
• Consult with physicians regarding change in treatment.

Compensation and Schedule:

• $38-$45 per visit
• Monday through Friday
• Weekends as needed
• PRN

What we’re looking for:

• A passion to serve and help others live their best lives possible.
• Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and /or certified by successfully completing the National Certification examination, -or-
• Eligible for the National Registration Examination of the American Occupational Therapy Association
• Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply
• Minimum of one year in an acute care setting.
• One (1) year experience in Home Health preferred.
