Job summary

Boost Home Healthcare is seeking an Occupational Therapist (OT) to administer timely in-home therapy under physician orders and the plan of care. You will assess, treat, and re-evaluate patients in their homes to maximize independence and safety during daily activities.

Qualifications

• Licensed OT in the practicing state or eligible for licensure.
• Eligible for the National Registration Examination of the American Occupational Therapy Association.
• Currently licensed by the state board where practicing, if licensure applies.
• Minimum of one year in an acute care setting.
• Two years experience in Home Health preferred.
• Access to reliable transportation.

Responsibilities

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and all skilled procedures.
• Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
• Consult with physicians regarding change in treatment.

Education

Graduate of an Occupational Therapy curriculum accredited by the American Occupational Therapy Association and/or National Certification examination

• Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply


• Minimum of one year in an acute care setting.
