Job summary

Winnie L Nursing & Rehab in Texas is seeking a Full-Time Occupational Therapist (OT) to join our in-house team. You will focus on improving residents’ independence and safety through meaningful daily activities and functional outcomes.

You will evaluate patients, develop personalized treatment plans, and collaborate with an interdisciplinary team. We value licensed OTs with SNF experience and knowledge of Net Health documentation.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily occupations, well-being, and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize performance in ADLs and IADLs.
• Assess patients’ home environments and recommend modifications based on needs and capabilities.
• Recommend adaptive equipment and provide training to support independence and safety.
• Ensure all treatments are delivered per the plan of care.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide clinical education and support to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely per policies and regulations.
• Attend required meetings designated by the Director of Rehab.
