Job summary

Winnie L Nursing in Texas is seeking a dedicated Occupational Therapist to enhance residents' independence and safety through meaningful ADLs and IADLs within a in-house rehab team.

The OT will assess conditions, develop individualized plans, modify environments, train staff and families, and document progress in Net Health, collaborating across disciplines to ensure high-quality patient care.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per the care plan and modify as needed in collaboration with the rehab team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation timely in compliance with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
