Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Franklin, MN. This full-time role offers a $5,000 sign-on bonus and tuition reimbursement, with Part Time and PRN options available.

The OT will plan, organize, and direct therapy services in the facility per federal and state standards, ensuring high-quality resident care and accurate documentation and billing. Active OT licensure and ongoing professional development are required.

Qualifikationen

• Must have an Active Occupational Therapy license.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Aufgaben

• Screen residents regularly to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals and methods to achieve them.
• Explain evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Create individual resident therapy schedules in collaboration with team members.
• Deliver therapy according to the treatment plan and ensure proper documentation and billing of services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet clinical excellence and reimbursement standards.
