Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Franklin, MN. This full-time role includes a $5,000 sign-on bonus and tuition reimbursement opportunities. Part-time and PRN positions are also available.

Plan, organize, develop, and direct OT services in the facility, ensuring compliance with standards and optimal resident care. Responsibilities include screening, evaluation, planning, documentation, and supervising COTA as needed.

Qualifications

• Licensed OT with degree in OT.
• Active OT license in the relevant state(s).
• Commitment to ongoing licensure and professional development.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform treatment plans; set short- and long-term goals and methods.
• Explain evaluation findings, treatment plans, and recommendations to residents, families, and the team.
• Create individual resident therapy schedules with the team.
• Provide therapy per plan and ensure documentation and billing.
• Perform other duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet clinical excellence and reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development
