Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Franklin, MN. This full-time role offers a $5,000 sign-on bonus, plus tuition reimbursement eligibility, with additional part-time or PRN positions available.

Responsibilities include planning, directing OT services, and coordinating with the interdisciplinary team to deliver high-quality resident care while ensuring proper documentation and billing.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform treatment plans and goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of OT services.
• Perform all other duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.
