Job summary

Evolve Therapy Services seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Franklin, MN. Full-time, with other options available, offering a $5,000 sign-on bonus and tuition reimbursement.

Responsibilities include planning and directing OT services to ensure high-quality resident care in accordance with standards. Qualified candidates hold an OT degree and active state licensure, with ongoing CEU participation.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop short- and long-term goals and plans.
• Communicate findings and plans to residents, families, and the interdisciplinary team.
• Schedule individual resident therapy sessions with team collaboration.
• Document and bill occupational therapy services accurately.
• Perform duties to effectively treat patients and ensure reimbursement.
• Oversee COTA and ensure compliant documentation and treatments.
