Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist to join an in-house skilled nursing facility in Olivia, MN. The role involves planning and directing OT services to ensure high-quality resident care.

The position offers full-time, part-time, or PRN options with a $5,000 sign-on bonus and tuition reimbursement, plus growth opportunities within a supportive interdisciplinary team. Licensure in the practicing state is required.

Qualifications

• Must hold active OT license in practicing state(s).
• Degree in Occupational Therapy required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to recommend interventions and treatments.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Establish individual therapy schedules with other team members.
• Conduct therapy per plan and document/bill therapy services accurately.
• Perform additional duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.
