Job summary

Lexington SNF Healthcare, LLC seeks an Occupational Therapist to evaluate residents, develop individualized treatment plans, and supervise assistants. You will communicate with families and physicians, contribute to care conferences, and ensure compliant documentation and safety in a post-acute setting.

You’ll educate patients and caregivers on home exercise programs, fall prevention, and equipment needs while collaborating with a diverse care team. Licensure and ongoing CE are required.

Qualifications

• Bachelor's degree in Occupational Therapy; Master or Doctorate preferred.
• Licensed Occupational Therapist in the state with active license.
• Continuing education as required by state practice acts.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise Occupational Therapy Assistants in resident care.
• Communicate progress and discharge plans with physicians, staff and families.
• Complete timely, accurate documentation per policies and regulations.
• Educate residents and families on home programs and safety.
• Assist with training Restorative Aides and participate in committee meetings.
