Job summary

Eureka of Cascadia LLC is seeking an Occupational Therapist to assess referred patients and develop hands-on rehabilitation plans in a long-term care setting. You will document patient progress and educate families, while coordinating with physicians and staff to optimize patient outcomes.

The role requires a state license as an OT, current CPR, and at least six months of long-term care experience; supervision of COTAs may be involved as part of the rehabilitation team.

Qualifications

• Must have graduated from an accredited school of occupational therapy.
• Current state license as an Occupational Therapist.
• Current CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Provide direct patient care and assess medical condition, functional capabilities, limitations, and rehabilitation potential.
• Develop and administer treatment programs with goals set by patient capacity and physician direction.
• Document patient status and progress; educate patients and families.
• Refer patients to CERTIFIED Occupational Therapy Assistants (COTA) when appropriate and supervise them.
• Coordinate with facility staff and participate in committee meetings and team rounds.
• Ensure punctuality and reliable attendance for scheduled shifts.
