Job summary

Summit Care LLC is seeking an Occupational Therapist to deliver high-quality therapy across patient care settings. You will evaluate patients, develop treatment plans, supervise assistants, and participate in case management to achieve measurable outcomes.

Responsibilities include screening, documentation, adherence to OT guidelines, and collaboration with facility staff to promote safety and quality of life for residents. Excellent communication and leadership are essential.

Qualifications

• Successful completion of an accredited Occupational Therapy Program
• Valid Occupational Therapy License to practice in appropriate state(s)
• Effective written and verbal communication skills
• Effective leadership and customer relations skills
• Ability to exercise mature judgment with initiative in analyzing and solving problems

Responsibilities

• Executes doctor’s orders
• Provides OT screening and evaluation to determine patient status
• Develops and implements treatment plans with functional outcomes
• Oversees maintenance of therapy records
• Ensures availability of supplies and equipment
• Maintains patient schedules per guidelines
• Documents for reimbursement and billing accuracy
• Participates in patient/family/facility meetings
• Follows safety and infection control policies
• Coordinates training programs for staff
• Initiates new program development to improve quality of life
• Participates in continuous quality improvement
• Lifts patients using biomechanical techniques as needed
• Walks, stands, and reaches during shifts
