Job summary

Summit Care LLC is seeking an Occupational Therapist to provide comprehensive OT services to patients, families, and facilities in accordance with therapy guidelines.

The role includes evaluation, treatment planning, discharge planning, and supervision of assistants, with adherence to state regulations and quality of care commitments. A sign-on bonus is offered, and home health experience may require driving and related certifications.

Qualifications

• Must have completed an accredited Occupational Therapy Program.
• Valid Occupational Therapy License to practice in the appropriate state(s).
• Excellent written and verbal communication skills.
• Demonstrated leadership and customer relations abilities.
• Ability to exercise mature judgment with initiative in analyzing and solving problems.
• Home health experience requires a current driver’s license, vehicle insurance, and First Aid/CPR certification.

Responsibilities

• Execute doctor’s orders and deliver OT evaluation and treatment.
• Develop and implement treatment plans with functional outcomes.
• Supervise OT assistants; maintain therapy records and documentation for billing and compliance.
• Ensure availability of supplies and equipment; manage patient schedules.
• Coordinate with patients, families, and facility staff; participate in meetings.
• Lead training for facility staff, residents, and families; participate in QA activities.
