Job summary

Willow Creek Senior Community is seeking a qualified rehabilitation professional to oversee patient care and guide therapy services within a comprehensive senior care setting.

You will develop individualized treatment plans, collaborate with an interdisciplinary team, and ensure accurate documentation and reporting while maintaining licensure and regulatory compliance. Strong emphasis on education for staff and coordination with family and caregivers.

Qualifications

• Experience documenting evaluations, progress notes, and reports.
• Ability to educate staff and third-party payers on rehab interventions.
• Must maintain state licensure and credentialing.

Responsibilities

• Initiates evaluations per MD orders and payer guidelines.
• Develops and delivers treatment plans to optimize functional outcomes.
• Educates staff and other healthcare professionals on intervention benefits.
• Completes documentation including evaluations, progress notes, and monthly reports.
• Directs and supervises assistants and rehab aides.
• Maintains safety and compliance with state and federal guidelines.
• Maintains licensure and credentialing.
• Coordinates referrals within rehab team.
