Job summary

Willow Creek Health Care Center is seeking a Full-Time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. The COTA will work under the supervision of a licensed Occupational Therapist to provide direct patient care and assist residents in achieving independence in daily activities within a skilled nursing setting.

The successful candidate will have a CA COTA license, experience in post-acute care, and strong skills in documentation, communication, and teamwork.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current, active California COTA license in good standing.
• Experience in a skilled nursing or post-acute setting preferred.
• Proficient in electronic medical records; PCC experience preferred.
• Strong knowledge of occupational therapy practices and rehabilitation techniques.
• Ability to follow treatment plans and adapt interventions.
• Effective communication and interpersonal skills.
• Strong organizational and documentation skills.
• Ability to work collaboratively in an interdisciplinary team.

Responsibilities

• Provide skilled occupational therapy treatments under supervision of the supervising Occupational Therapist.
• Document daily treatment notes, weekly progress notes, and other required records.
• Communicate resident progress, concerns, and treatment updates to the OT and interdisciplinary team.
• Assist with discharge planning and caregiver education.
• Instruct residents, families, and nursing staff in maintenance programs and safety techniques.
• Participate in interdisciplinary care conferences as needed.
• Maintain cleanliness and organization of therapy areas and equipment.
• Report equipment issues to ensure safe and effective operation.
• Maintain professional and respectful communication with residents, families, and team members.
