Job summary

Willow Creek Health Care Center is seeking a dedicated Full-Time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a skilled nursing setting. The COTA will work under the supervision of an Occupational Therapist to provide direct patient care and help residents achieve independence in daily activities, mobility, and function.

The role emphasizes collaboration with an interdisciplinary team, adherence to treatment plans, and accurate documentation.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current, active, and unencumbered COTA license in the State of California.
• Experience in a skilled nursing or post-acute setting preferred.

Responsibilities

• Provide skilled occupational therapy treatments as directed by the supervising Occupational Therapist.
• Carry out therapy interventions in accordance with physician orders and treatment plans.
• Document daily treatment notes, weekly progress notes, and other required records.
• Communicate resident progress and treatment updates to the Occupational Therapist and interdisciplinary team.
• Assist with discharge planning and caregiver education.
• Instruct residents, families, and nursing staff in maintenance programs and safety techniques.
• Participate in interdisciplinary care conferences as needed.
• Maintain cleanliness and organization of therapy areas and equipment.
• Report equipment issues to ensure safe operation.
• Maintain professional communication with residents, families, and team members.
