Job summary

Alpha Home Health and Hospice seeks an Occupational Therapist to assess residents, plan rehabilitative programs, and implement interventions that promote independence in daily activities.

You will conduct comprehensive evaluations of physical, cognitive, and psychosocial functioning, develop individualized treatment plans, document progress, and collaborate with a multidisciplinary team to coordinate care and discharge planning.

Qualifications

• Current state licensure or eligibility as an Occupational Therapist.
• Preferred experience in skilled nursing or similar healthcare settings.
• Compassionate, patient-centered approach with strong interpersonal skills.
• Proficiency in assessment, treatment planning, and intervention techniques.

Responsibilities

• Conduct assessments of residents’ physical, cognitive, and psychosocial functioning.
• Develop individualized treatment plans aligned with resident goals and physician recommendations.
• Provide interventions such as ADL training, therapeutic exercises, adaptive equipment training, and environmental modifications.
• Document progress, interventions, and discharge plans following facility policies and regulations.
• Collaborate with healthcare professionals to ensure coordinated resident care.
• Educate residents and families on therapeutic techniques, safety, and home programs.
• Participate in team meetings to contribute to care planning and goal setting.
• Stay current with evidence-based practices and regulatory updates in occupational therapy.
• Maintain licensure and adhere to professional ethical standards.
• Demonstrate professionalism, teamwork, and effective communication to foster a positive work environment.
