Job summary

Elm Management is seeking a compassionate Occupational Therapist (OTR) to evaluate, plan, and implement individualized treatment programs to help patients achieve maximum independence and quality of life.

The role collaborates with physicians, nursing staff, and the inter-disciplinary team to address ADLs, IADLs, mobility, cognition, and lymphedema management, while documenting progress, discharge summaries, and ensuring compliance with state regulations and facility procedures.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current state Occupational Therapist license (OTR).
• Lymphedema Management certification/experience.
• Strong communication, time management, and organization skills.

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Provide skilled occupational therapy services in accordance with physician orders and plan of care.
• Address ADLs, IADLs, mobility, strength, coordination, cognition, and adaptive equipment needs.
• Educate patients, families, and caregivers on treatment goals, progress, and home programs.
• Document evaluations, progress notes, and discharge summaries accurately and timely.
• Collaborate with physicians, nursing staff, and interdisciplinary team members.
• Maintain compliance with state regulations, facility procedures, and ethical standards.
