Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking a PRN Occupational Therapist (OTR) to join a dynamic rehab team. The role welcomes new grads and offers flexible PRN scheduling, with a pay range of $57–$67 per hour. The incumbent will develop individualized plans, provide OT services per orders, and support staff training.

Responsibilities

• Develop individualized treatment plans and obtain physician approval
• Provide occupational therapy services per physician orders
• Assist nursing staff with training Restorative Aides
• Supervise Occupational Therapy Assistants in accordance with state practice acts
• Communicate patient progress, challenges, and care plans with the interdisciplinary team
• Participate in patient care conferences, utilization review, and rehabilitation meetings as needed
