Job summary

Sunnyvale Post Acute Center is seeking an Occupational Therapist/OTR to join our on-site therapy team in Sunnyvale, CA. We offer full-time, part-time and PRN opportunities, with a competitive hourly rate and a supportive mentoring atmosphere for new grads.

Develop treatment plans, treat patients per physician orders, supervise OT Assistants, and collaborate with nurses and staff. We provide modern equipment, flexible scheduling, and comprehensive benefits to help you grow professionally.

Qualifications

• Valid OT license required.
• OTR certification.
• New grads welcome.

Responsibilities

• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Supervise Occupational Therapy Assistants in direct patient care and patient related activities, following state practice act.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.
