Sunnyvale Gardens Post Acute is seeking a PRN Occupational Therapist (OTR) to join our rehabilitation team in Sunnyvale, CA. The role offers flexible scheduling and a pay range of $57-$67 per hour for PRN coverage.

• Current and valid Occupational Therapy License (OTR) in good standing.

Responsibilities

• Develop individualized treatment plans and obtain physician approval.
• Provide occupational therapy services per physician orders.
• Assist nursing staff with training Restorative Aides.
• Supervise Occupational Therapy Assistants in accordance with state practice acts.
• Communicate patient progress, challenges, and care plans with the interdisciplinary team.
• Participate in patient care conferences, utilization review, and rehabilitation meetings as needed.
