Job summary

Nexushm is seeking an Occupational Therapist in Belleville, IL to evaluate and treat residents, helping them regain and improve daily living skills. You will create personalized plans and collaborate with the IDT to optimize safety and independence.

The role emphasizes timely documentation, adherence to care plans, and ongoing assessment of patient progress in a SNF/long-term care setting, with a focus on clinical excellence and compassionate care.

Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Evaluate and treat residents to regain, maintain, or improve daily living skills.
• Develop personalized treatment plans using techniques to maximize ADL/IADL safety and performance.
• Ensure all treatment aligns with the established plan of care and is properly documented.

Position Overview:

Evaluate and treat residents to help them regain, maintain, or improve the skills needed for daily living and functional independence.

Key Responsibilities:

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
