Nexushm is seeking an Occupational Therapist to evaluate and treat residents to help them regain, maintain, or improve daily living and functional independence in a SNF setting.

Responsibilities include developing personalized treatment plans, ensuring care is aligned with the plan, monitoring responses, and documenting thoroughly in PCC Electronic Documentation System, while collaborating with the interdisciplinary team.

Qualifikationen

• Active Illinois license in Occupational Therapy.
• Current knowledge of treatment practices.
• SNF/Long term care experience preferred.
• Knowledge of PCC Electronic Documentation System preferred.

• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
