Nexus Health is seeking an Occupational Therapist to evaluate and treat residents to help them regain, maintain, or improve daily living skills and functional independence.
You will develop personalized treatment plans and monitor patient responses, collaborating with the IDT to adjust plans while ensuring compliant documentation and regulatory adherence. A license in Illinois is required, with SNF/long-term care experience preferred.
Position Overview:
Evaluate and treat residents to help them regain, maintain, or improve the skills needed for daily living and functional independence.
Key Responsibilities:
Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
Ensure all treatment delivered is in accordance with the established plan of care.
Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

Preferred Qualifications:
Active/Valid Illinois license in Occupational Therapy
SNF/Long term care experience preferred.
Current knowledge of treatment practices
Knowledge of PCC Electronic Documentation System preferred.
