Job summary

Nexushm is seeking an Occupational Therapist to evaluate and treat residents to regain, maintain, or improve skills for daily living and functional independence.

You will develop individualized treatment plans and collaborate with the IDT to adjust approaches while ensuring compliant documentation and regulatory adherence.

Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans to maximize safety and performance in ADL/IADL.
• Ensure all treatment delivered aligns with the established plan of care.
• Monitor patient response and modify treatment with IDT collaboration.
• Ensure documentation is thorough, timely, and compliant with regulations.

Position Overview:

Evaluate and treat residents to help them regain, maintain, or improve the skills needed for daily living and functional independence.

Key Responsibilities:

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
