Job summary

Anova Care, a provider of home care and home health services, is seeking an Occupational Therapist to join our growing home health program. You will render home health services to patients in their homes, conducting evaluations and delivering personalized OT interventions.

Qualifications

• Master’s or Doctorate in Occupational Therapy required.
• Current state licensure as an Occupational Therapist.
• 1–3 years clinical experience in home health, outpatient, or rehabilitation settings preferred.
• Strong assessment, treatment planning, and patient education skills.
• Knowledge of home health regulations and documentation requirements.
• Proficiency with EMR systems and Microsoft Office Suite.
• Excellent written and verbal communication and interpersonal skills.
• Ability to work independently and manage a patient caseload.
• Availability during business hours with occasional after-hours/weekend coverage.

• Conduct patient evaluations and develop individualized OT treatment plans
• Provide skilled occupational therapy interventions and patient education in the home setting

• Communicate effectively with patients, families, and interdisciplinary teams
• Ensure compliance with home health regulations, physician orders, and documentation standards

Position Qualifications

Hours: PRN, Pay-per-visit
