Job summary

Heartland Nursing and Rehabilitation Center in Boynton Beach, FL is seeking a compassionate Occupational Therapist to join our rehabilitation team on a PRN basis. You will evaluate residents, develop individualized treatment plans, and apply evidence-based interventions to improve daily living skills and promote independence.

Collaborate with physicians, nurses, and other therapists in a supportive, team-oriented environment.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and active Florida Occupational Therapist license.
• Clear/active license and Level 2 background screening.
• Experience in long-term care or skilled nursing facilities preferred.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans based on residents' functional goals.
• Utilize evidence-based therapeutic interventions to improve independence in activities of daily living.
• Monitor resident progress and modify treatment plans as necessary.
• Maintain accurate, timely, and compliant documentation.
• Collaborate with an interdisciplinary team to ensure coordinated and effective care.
• Assist residents recovering from illness, injury, or disability in maximizing their functional potential.
• Promote a positive, resident-centered care environment.
