Job summary

Heartland Nursing and Rehabilitation Center in Boynton Beach, Florida, is seeking a compassionate Occupational Therapist for a PRN position. You will assess residents, develop individualized treatment plans, and use evidence-based interventions to promote independence in activities of daily living.

You will collaborate with physicians, nurses, and other therapists in a supportive team to monitor progress, adjust plans, and document outcomes in a compliant, timely manner.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and active Florida Occupational Therapist license.
• Clear/active license and Level 2 background screening.
• Experience in long-term care or skilled nursing facilities preferred.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans based on residents' functional goals.
• Utilize evidence-based therapeutic interventions to improve independence in daily living activities.
• Monitor resident progress and modify treatment plans as necessary.
• Maintain accurate, timely, and compliant documentation.
• Collaborate with an interdisciplinary team to ensure coordinated and effective care.
• Assist residents recovering from illness, injury, or disability in maximizing their functional potential.
• Promote a positive, respectful, and resident-centered care environment.
