Anova Care is seeking an Occupational Therapist (OT) to join our growing home health program in Illinois. You will render home health services, conduct patient evaluations, and develop individualized treatment plans in patients' homes.

The role requires a Master’s or Doctorate in Occupational Therapy and current state licensure. 1–3 years of related experience is preferred. You will document progress in the EMR and communicate with patients and care teams. PRN, pay-per-visit arrangement offered.

Key Responsibilities

• Conduct patient evaluations and develop individualized OT treatment plans
• Provide skilled occupational therapy interventions and patient education in the home setting
• Monitor and document patient progress and outcomes in the EMR
• Communicate effectively with patients, families, and interdisciplinary teams
• Ensure compliance with home health regulations, physician orders, and documentation standards
• Participate in quality improvement initiatives and care coordination meetings
• Collaborate with clinical, administrative, and operations teams to support patient care and service delivery
• Perform other clinical or administrative duties as assigned

Position Qualifications

• Master’s or Doctorate in Occupational Therapy required
• Current state licensure as an Occupational Therapist
• 1–3 years clinical experience in home health, outpatient, or rehabilitation settings preferred
• Strong assessment, treatment planning, and patient education skills
• Knowledge of home health regulations and documentation requirements
• Proficiency with EMR systems and Microsoft Office Suite
• Excellent written and verbal communication and interpersonal skills
• Ability to work independently and manage a patient caseload
• Availability during business hours with occasional after-hours/weekend coverage

Hours: PRN, Pay-per-visit

Pay: $70.00 to $130.00
