Flagship Therapy seeks a licensed Occupational Therapist to join the Mt. Angel in-house rehab program in Mt. Angel, OR. This full-time role focuses on cognitive/ADL assessments and interventions for geriatric post-acute care patients.

You will apply evidence-based treatments, collaborate with a health-team, and contribute to discharge planning and family counseling as part of comprehensive patient care.

Qualificações

• Licensed OT in good standing.
• Open to applicants of all experience levels.

Responsabilidades

• Screen/evaluate patients with cognitive and ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative and medically necessary treatment per clinical requirements.
• Administer ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed based on results.
• Provide counseling to patients and families and other providers.
• Communicate progress, barriers, and plans with the health team.
• Prepare comprehensive discharge summaries per regulatory requirements.
