Job summary

Simpsonville Post Acute is seeking a compassionate Occupational Therapist (OTR) to deliver patient-centered therapy and improve residents' independence and quality of life. You will evaluate residents, plan treatments, and collaborate with the interdisciplinary care team.

The role requires a degree in OT, an active license in SC, and experience in skilled nursing or rehab. PCC and Casamba familiarity are preferred; excellent documentation and communication are essential.

Qualifications

• Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred).
• Active South Carolina Occupational Therapist (OTR) license (in good standing).
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare (PCC) and Casamba preferred.
• Strong clinical, documentation, and communication skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy services per physician orders and care goals.
• Document evaluations, treatments, and progress notes accurately and timely.
• Communicate with physicians, nursing staff, and interdisciplinary team members.
• Educate residents, families, and caregivers on therapy programs.
• Participate in care planning and discharge planning.
• Conduct staff in-services and training.
• Maintain compliance with all state, federal, and payer regulations.
