This role focuses on evaluation, treatment planning, and collaboration with the interdisciplinary care team.### **Key Responsibilities*** Evaluate residents and develop individualized **treatment plans*** Provide therapy services per **physician orders and care goals*** Document evaluations, treatments, and progress notes accurately and timely* Communicate with physicians, nursing staff, and interdisciplinary team members* Educate residents, families, and caregivers on therapy programs* Participate in **care planning and discharge planning*** Conduct **staff in-services and training*** Maintain compliance with all **state, federal, and payer regulations**### **Qualifications*** Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred)* Active **South Carolina Occupational Therapist (OTR) license** (in good standing)* Experience in **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba** preferred* Strong clinical, documentation, and communication skillsWe are an equal opportunity employer.
