Job summary

White Blossom Care Center in San Jose, CA is seeking an Occupational Therapist (OTR) to join our in-house rehabilitation program. This position offers a high-volume, resource-rich clinical environment with access to a large rehab gym and advanced modalities.

We welcome new graduates and offer full-time, part-time, or PRN schedules. Eligible candidates must hold a California OT license in good standing and be committed to teamwork and patient-centered outcomes.

Qualifications

• California Occupational Therapist license in good standing.
• Passion for delivering high-quality care in post-acute and long-term care settings.

Responsibilities

• Evaluation & Treatment: perform comprehensive evaluations and provide OT interventions per physician orders.
• Clinical Leadership: supervise and support COTAs per California State Practice Acts.
• Interdisciplinary Collaboration: participate in care conferences and rehab meetings for coordinated care.
• Documentation Excellence: complete evaluations, daily notes, progress notes, and recertifications per OT Board standards.
• DME & Patient Independence: recommend, order and train patients on durable medical equipment to promote safety.
• Education & Discharge Planning: provide caregiver training to families and nursing staff for positive discharge outcomes.

Key Responsibilities

• Evaluation & Treatment: Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions per physician orders
