Job summary

White Blossom Care Center in San Jose, CA is seeking a licensed Occupational Therapist (OTR) to join our premier in-house rehabilitation program. Join a resource-rich environment with the Bay Area's largest rehab gym and access to advanced modalities to support patient independence.

As part of a collaborative team, you will evaluate and treat, supervise COTAs, participate in care conferences, and guide caregivers to optimize discharge outcomes.

Qualifications

• California OT license in good standing.
• Experience in post-acute and long-term care settings is preferred.
• Ability to collaborate with a multidisciplinary team and focus on patient-centered outcomes.

Responsibilities

• Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions per physician orders.
• Supervise and support Certified Occupational Therapy Assistants (COTAs) in compliance with California State Practice Acts.
• Participate in care conferences, utilization reviews, and rehab meetings to ensure coordinated, patient-centered care.
• Complete timely and accurate evaluations, daily notes, progress notes, and recertifications per OT Board standards.
• Assess DME needs, order equipment, and train patients for safety and independence.
• Provide caregiver training and education to families and nursing staff to support positive discharge outcomes.
