Job summary

Centra Healthcare Solutions is seeking an Occupational Therapist (OT) for a contract assignment at a skilled nursing facility in McMinnville, Oregon. Both experienced and new graduates are welcome; you will provide skilled OT services under physician orders and collaborate with the care team.

Must have graduated from an ACOTE-accredited program, hold a current OT license (or be in process) and maintain CPR certification.

• Graduated from ACOTE-accredited OT program.
• Current state OT license or in process for licensure.
• Current CPR certification.

Responsibilities

• Provide skilled occupational therapy services per physician orders.
• Assess patient needs and plan, evaluate, and modify care for OT goals.

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment! This is a contract position. Days schedule.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Current CPR certification.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
