Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a dedicated Occupational Therapist to deliver resident-centered care in a skilled nursing and rehab setting. You will evaluate functional abilities, plan individually tailored therapies, and provide direct OT services per physician orders.

The role requires CA licensure, EMR experience (Casamba and PointClickCare a plus), and collaboration with nursing staff and therapy teammates.

Qualifications

• Licensed OT in California with active CA license.
• Experience with EMR systems (Casamba and PointClickCare a plus).
• SNF or post-acute rehab experience preferred but not required.

Responsibilities

• Evaluate residents to determine functional abilities and therapy needs.
• Develop and implement individualized treatment plans.
• Provide direct therapy services per physician orders.
• Supervise COTAs and collaborate with the interdisciplinary care team.
• Document treatment progress and outcomes in compliance with regulatory standards.
• Participate in discharge planning and caregiver education.
• Contribute to in-service training and quality improvement initiatives.
