Job summary

Oakwood Gardens Care Center in Fresno, CA seeks a licensed Physical Therapist to evaluate residents and develop individualized plans. You will provide direct PT treatments, document progress, and collaborate with an interdisciplinary team to ensure comprehensive care.

You will supervise PTAs, participate in discharge planning, and educate caregivers while maintaining regulatory compliance. This is a full-time on-site role with competitive pay and comprehensive benefits.

Qualifications

• Licensed Physical Therapist (PT) in California.
• Strong clinical skills and dedication to resident-centered care.
• Ability to work independently as well as part of a team.
• Excellent communication and organizational skills.
• Skilled nursing or post-acute rehab experience preferred, but not required.

Responsibilities

• Evaluate residents to determine therapy needs and create individualized treatment plans.
• Provide direct physical therapy treatments per physician orders and resident goals.
• Document evaluations, progress notes, and treatment records accurately and timely.
• Collaborate with an interdisciplinary team to ensure comprehensive resident care.
• Supervise and support Physical Therapy Assistants (PTAs).
• Participate in discharge planning and caregiver education for continuity of care.
• Maintain compliance with state, federal, and facility regulations.
