Job summary

The OT also provides education and in-services to facility staff to support optimal resident outcomes.### **Essential Duties and Responsibilities*** Evaluate residents within **24 hours (or one business day)** of physician referral; document any delays appropriately* Develop individualized treatment plans and obtain physician approval* Treat residents according to physician-approved plans of care* Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in compliance with state practice act* Assist nursing staff with training of Restorative Aides as appropriate* Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams* Participate in patient care conferences, utilization review meetings, and rehab conferences as needed* Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements* Educate residents, families, and caregivers on maintenance programs and discharge preparation* Participate in discharge planning and care coordination* Recommend and facilitate ordering of durable medical equipment (DME) to promote independence in activities of daily living* Report equipment issues to ensure safe and effective operation* Maintain positive, professional relationships with residents, families, staff, and colleagues### **Supervisory Responsibilities*** Supervise **Certified Occupational Therapy Assistants (COTAs), aides, and students**### **Qualifications****Education & Experience*** Bachelor’s degree in Occupational Therapy required* Master’s or Doctorate degree in Occupational Therapy preferred* Skilled nursing or geriatric experience preferred**Licensure & Certifications*** Current, active **California Occupational Therapist license** in good standing* Must meet all continuing education requirements per state regulations**Skills*** Proficient in electronic documentation systems (**Casamba and PointClickCare preferred**)* Strong clinical judgment, communication, and organizational skills* Ability to work independently and collaboratively### **Physical Demands*** Frequent standing, walking, reaching, pushing, and pulling* Ability to exert up to **50 pounds frequently** and **100 pounds occasionally*** Occasional climbing, balancing, stooping, kneeling, crouching, or crawling* Visual abilities include close, distance, color, peripheral, and depth perception* Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions### **Work Environment*** Skilled nursing and rehabilitation setting* Noise level typically low to moderate* Reasonable accommodations available### **Additional Information**This job description does not restrict management’s right to assign or reassign duties at any time.
