Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a full-time Occupational Therapist (OT) to join our skilled nursing and rehab team focused on resident-centred care and improving quality of life.

You will evaluate residents, develop and implement individual treatment plans, provide direct therapy, supervise COTAs, document progress, and participate in discharge planning and caregiver education as part of an interdisciplinary team.

Qualifications

• California OT license required.
• Experience in SNF or post-acute rehab preferred.
• Strong communication and collaboration with nursing staff and therapy team.

Responsibilities

• Evaluate residents to determine functional abilities and therapy needs.
• Develop and implement individualized treatment plans.
• Provide direct therapy services in accordance with physician orders.
• Supervise COTAs and collaborate with the interdisciplinary care team.
• Document treatment progress and outcomes in compliance with regulatory standards.
• Participate in discharge planning and caregiver education.
• Contribute to in-service training and quality improvement initiatives.

What We're Looking For:

• A licensed Occupational Therapist (OT) in the State of California
• Someone who brings compassion, clinical expertise, and strong communication skills
