Job summary

Garden City Health Care Center in Modesto, CA is seeking a compassionate Occupational Therapist (OT) for a full-time position in our skilled nursing facility. You will evaluate residents, develop individualized plans, and supervise COTAs and aides to support resident independence and quality of life.

You will collaborate with families, physicians, and the rehab team, complete timely documentation, and participate in discharge planning.

Qualifications

• Bachelor's degree in Occupational Therapy is required.
• Master's or Doctorate in Occupational Therapy preferred.
• Current California OT license in good standing is required.
• Experience in skilled nursing or geriatric settings preferred.

Responsibilities

• Evaluate residents within 24 hours of referral and document delays.
• Develop individualized plans of care and obtain physician approval.
• Supervise COTAs, aides, and students.
• Assist with training Restorative Aides as needed.
• Provide education to residents, families, and staff.
• Participate in discharge planning and care coordination.
• Complete required documentation (evaluations, notes, progress, recertifications).
