Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a full-time Occupational Therapist to evaluate residents, develop individualized treatment plans, and provide direct therapy services in coordination with physicians and an interdisciplinary team.

The role emphasises collaboration with nursing staff, therapy assistants, and other healthcare professionals to improve patient independence and quality of life.

Qualifications

• Licensed OT in California required.
• Experience with EMR systems preferred (Casamba and PointClickCare a plus).
• SNF or post-acute rehab experience preferred but not required.

Responsibilities

• Evaluate residents to determine functional abilities and therapy needs.
• Develop and implement individualized treatment plans.
• Provide direct therapy services in accordance with physician orders.
• Supervise COTAs and collaborate with the interdisciplinary care team.
• Document treatment progress and outcomes in compliance with regulatory standards.
• Participate in discharge planning and caregiver education.
• Contribute to in-service training and quality improvement initiatives.
