Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a compassionate Per Diem Occupational Therapist (OT) to join our skilled rehab team and help residents regain independence. You will evaluate residents, develop individualized treatment plans, and deliver interventions targeting ADLs, mobility, safety, and cognition while educating families and staff.

California OT licensure is required; Master’s/Doctorate is preferred.

Qualifications

• Bachelor’s Degree in Occupational Therapy is required.
• California OT license in good standing is required.
• Master’s or Doctorate in OT is preferred.
• Experience with Casamba and PointClickCare is a plus.
• Excellent communication and teamwork abilities are essential.

Responsibilities

• Evaluate residents and determine OT needs and rehab potential.
• Develop, implement, and modify individualized treatment plans.
• Provide interventions focused on ADLs, mobility, safety, cognition, and balance.
• Educate residents, families, and caregivers on home programs and fall prevention.
• Participate in care plan meetings and interdisciplinary conferences.
• Document progress timely and accurately; ensure regulatory compliance.
• Supervise COTAs and therapy aides as appropriate.
