Linwood Meadows Care Center in Visalia, CA is seeking a compassionate Occupational Therapist (OT) to join our rehab team on a full-time basis. The role offers competitive pay and a supportive, resident-centered culture aimed at promoting independence and quality of life.

The OT will evaluate residents, create individualized treatment plans, and provide interventions to improve ADLs, mobility, safety, and overall functional performance.

Qualifications

• Bachelor’s Degree in Occupational Therapy is required.
• Master’s or Doctorate in OT preferred.
• California OT license in good standing.

Responsibilities

• Evaluate residents and determine OT needs and rehab potential.
• Develop, implement, and modify individualized treatment plans.
• Provide therapies focused on ADLs, mobility, balance, cognition, and safety.
• Educate residents and caregivers on home programs and fall prevention.
• Supervise COTAs and therapy aides as appropriate.
• Complete timely, accurate, and compliant documentation.
• Collaborate with physicians and nursing staff on goals.
• Maintain a safe treatment environment and infection control.

General Purpose

The Occupational Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions to improve independence with daily living skills, mobility, safety, and overall functional performance.

Essential Duties

• Evaluate residents and determine occupational therapy needs and rehabilitation potential
• Develop, implement, and modify individualized treatment plans
• Provide therapeutic interventions focused on ADLs, strength, coordination, balance, cognition, and safety
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals
• Participate in care plan meetings and interdisciplinary team conferences
• Complete timely, accurate, and compliant documentation
• Educate residents and caregivers on home programs, adaptive strategies, and fall prevention
• Recommend adaptive equipment and assistive devices to promote independence
• Supervise COTAs and therapy aides as appropriate
• Maintain a safe treatment environment and follow infection control procedures

Qualifications

• Education: Bachelor’s Degree in Occupational Therapy required
• Master’s or Doctorate preferred
• License: Current California Occupational Therapy license in good standing
