Job summary

Evolve Therapy Services is seeking an Occupational Therapist to provide in-house OT services at a Skilled Nursing Facility in Wabasso, Minnesota. You will assess residents, develop treatment plans, and collaborate with an interdisciplinary team to maximize resident outcomes.

The role requires an active OT license in the practicing state, ongoing education, and a commitment to clinical excellence, documentation, and compliant billing.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy and ongoing continuing education.
• Commitment to professional development and licensure maintenance.

Responsibilities

• Conduct regular screening of residents to identify interventions.
• Evaluate residents to inform treatment plans and goals.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish individual therapy schedules with the care team.
• Deliver OT services per plan with proper documentation and billing.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
• Perform other duties as assigned to support resident care.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
