Job summary

Evolve Therapy Services in Olivia, MN is seeking an experienced Occupational Therapist to lead in-house therapy at a Skilled Nursing Facility partner. The role emphasizes delivering high-quality OT services aligned with regulatory standards.

We offer Full Time, Part Time, or PRN positions with a $5,000 sign-on bonus and tuition reimbursement. Candidates must hold an Active OT license and participate in ongoing professional development.

Qualifications

• Must hold an Active Occupational Therapy license in the practicing state.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
