Job summary

Evolve Therapy Services in Olivia, MN seeks a compassionate Occupational Therapist to join our in-house team at a Skilled Nursing Facility. Full-time, part-time, or PRN positions are available, with a $5,000 sign-on bonus plus tuition reimbursement.

You will screen residents, develop and adjust treatment plans, and work with families and an interdisciplinary team to ensure high-quality resident care. Supervision of COTA and accurate documentation are part of the role.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
