Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house skilled nursing facility in Wabasso, MN. This full-time opportunity may include part-time or PRN options along with a $5,000 sign-on bonus and tuition reimbursement.

You will plan, organize, develop, and direct OT services in accordance with applicable standards and regulations, ensuring high-quality resident care. Collaboration with an interdisciplinary team is essential to success.

Qualifications

• Active OT license in practicing state(s) is required.
• Degree in Occupational Therapy is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to identify the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Establish resident therapy schedules in collaboration with the team.
• Conduct therapy per plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and treatments follow reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient


• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals


• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members


• Establish individual resident therapy schedules in cooperation with other team members


• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services


• Perform all other duties as assigned and as required to effectively treat patients


• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices being followed

Education & Qualifications:

• Degree in Occupational Therapy


• MUST have an Active Occupational Therapy license in practicing state(s)


• Ongoing continuing education and professional development to maintain current licensure and certification
