Evolve Therapy Services is recruiting a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Wabasso, MN. This full-time role offers a $5,000 sign-on bonus plus tuition reimbursement, with additional Part Time or PRN options available.

The therapist will plan, organize, and direct OT services to ensure high-quality resident care in compliance with federal, state, and local regulations, while supervising the COTA and collaborating with an interdisciplinary team to

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsabilités

• Screen residents to determine interventions and treatments.
• Evaluate residents and develop treatment plans with goals.
• Interpret and communicate findings to residents, families, and the team.
• Establish individual therapy schedules with the team.
• Deliver OT services according to plan and document/bill appropriately.
• Oversee COTA and ensure compliant documentation and reimbursement.

Formation

Degree in Occupational Therapy

Active OT license in practicing state(s)

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
