Job summary

Evolve Therapy Services, LLC in Franklin, MN is seeking an Occupational Therapist for an in-house Skilled Nursing Facility. The role supports full-time, part-time, and PRN positions with a focus on delivering high-quality therapy services in a partnered facility.

The Occupational Therapist will assess residents, develop and implement treatment plans, coordinate with the care team, and ensure proper documentation and billing. Active licensure is required.

Qualifications

• Degree in Occupational Therapy.
• Must hold an active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
