Job summary

Evolve Therapy Services is recruiting a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Wabasso, MN. This full-time role includes a $5,000 sign-on bonus and tuition reimbursement, with additional part-time/PRN options.

Responsibilities include planning, organizing, and directing OT services, developing treatment plans, and ensuring compliance with CMS documentation standards. Must have OTA licensure and an associate degree.

Qualifications

• Must hold OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Strong documentation and communication skills.

Responsibilities

• Conduct regular resident screenings to determine best interventions.
• Execute treatment plans and monitor progress with supervising therapist guidance.
• Interpret and communicate evaluation findings to residents, families, and the care team.
• Establish individual resident therapy schedules with the team.
• Document and bill for occupational therapy services per plan of care.
• Perform duties necessary to effectively treat patients.
• Adhere to CMS documentation standards daily.

Job Position

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time or PRN positions available

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice

• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy

• MUST have active OTA licensure in state(s) of practice

• Ongoing continuing education and professional development to maintain current licensure and certification
